This initial assessment questionnaire allows you to have a better idea of how to reach your goals.

1) The first step is to fill out the form the best you can and make sure you fill every area in or you will receive a message before it is submitted to us.

2) The second step is to review “the 177 things about you” multi-page Initial Assessment Report that you will receive within 48 hours of submitting your information.

3) The Third step is to review the results. Come up with a plan to achieve your goals you set over a valid time period.
